HEENT:  Denies any tinnitus, ear pain, or drainage.  She also denies any upper respiratory symptoms. She does note occasional tension headaches.

Respiratory:  Has occasional dyspnea on exertion, especially going up stairs.

Cardiovascular:  Denies palpitations, Chest pain or syncope.

GI:  Unremarkable

GU: unremarkable

Psychiatric:  Denies symptoms of depression.  Has no unusual stressors.

Neurologic:  No focal weakness or numbness.  Denies diplopia, dysphagia  and dysarthria.

Hematologic:  Denies any blood loss or history of anemia
