HEENT:
Eyes: Pupils round and reactive to light, no nystagmus is appreciated. Her ophthalmoscopic examination was normal.
Ears: Clear canals and TMs. Hearing is grossly normal
Oropharynx: Normal
Neck: Carotid pulsations are equal an full without bruits, no adenopathy
Lungs: Clear
Cardiac: Regular rhythm with a normal S1 and S2. No murmurs are noted.
Neurological: Cranial nerves are grossly intact. She demonstrates adequate finger-to-nose, heal-to-shin and rapid alternating maneuvers. The Romberg test is negative. Sensation and strength appear intact. Her gait is unremarkable. DTR's are equal at 2/4 throughout.
Dix-Hallpike maneuver demonstrates rotational nystagmus with her right ear down. The patient becomes quite symptomatic and sits up. The maneuver is repeated several times, however the nystagmus and symptoms do not recur.
